Choices for Care

The VNA program offers case management and personal care service to adults with physical disabilities. Individuals must qualify for Level II nursing home placement and meet financial eligibility criteria. Assistance with personal care, activities of daily living, and homemaker tasks are provided. Individual care plans are designed for clients to address individual needs and goals.

VNA Case Management and Choices for Care

VNA Case Managers are nurses or social workers who use their experience and knowledge to help you best meet your needs. Your Case Manager will work to bring help and relief into your home and to prevent problems by thinking of things that you don’t. Working as a team, we ensure that you are receiving the services you need from the VNA or other agencies.

Once you choose the VNA to provide case management, your VNA Case Manager will call you to talk about what you want from Choices for Care and to begin discussing financial paperwork.

Your Case Manager will meet with you in person and complete an initial assessment to determine how many hours of care you are eligible to receive. Your Case Manager will help you identify other services and benefits for which you may be eligible – such as transportation or equipment – and then help you put them in place.

Once you are part of the Choices for Care program, your VNA Case Manager will visit you at least once a month and can be called at any time.

VNA Caregivers and Choices for Care

Before providing care to you or your loved one, VNA caregivers are carefully trained by a registered nurse and a physical therapist. They are trained to meet your individual needs by a nurse who will come to your home to work with you to create your plan of care. Your caregivers are supervised by your VNA nurse who will visit with you at least every other month to ensure your plan of care is best meeting your needs.
