End-of-Life Care

The VNA provides support for those living with a life-threatening illness or caring for a loved one with a life-limiting disease. The compassionate end-of-life care experts of our Hospice of the Champlain Valley provide:
- Nursing and personal care,
- Psycho-social, spiritual, and emotional support,
- Pain and symptom management for individuals early on in their illness, through the last days of life and during the dying process,
- Bereavement care for families.
Whether it is in a home, a nursing home or in our Vermont Respite House, we provide services to help support patients and their families as they face the multitude of issues surrounding end-of-life care.
The VNA offers a range of hospice and palliative care programs to help achieve comfort, promote choice and provide support.
These programs include:
- Hospice Care — Hospice of the Champlain Valley provides care to those in the final stages of life.
- Palliative Care — Services to help those with a life-threatening or life-limiting illness maintain the highest quality of life possible.
- Vermont Respite House — A home away from home for people with terminal illness where they can live in peace surrounded by the love of family and friends.
- Madison-Deane Initiative — A community outreach program providing education and resources.
- Grief & Bereavement Programs — Offering counseling, support groups, an annual memorial service and a weekend family bereavement camp.
- Pediatric Supportive Care — Palliative care services for children with life-limiting or life-threatening illnesses.
