As your home care provider, it is important we have the most up-to-date insurance information on record.
Your updated information allows us to contact your insurance company or your Medicare HMO to verify that our services will be covered under your policy and to obtain any required authorizations prior to home care visits.
You can provide your updated information to us in one of two ways:
Patients may be liable for charges incurred if it is determined we were not informed of changes in a timely manner. Although we will make every attempt to ensure our information is current, ultimately it is the patient's responsibility to inform their health care provider when their coverage has changed.
It is important to note that regardless of any changes to your policy or carrier, you may need to meet new deductibles and co-pays in the new calendar year.
