Making a Referral and Arranging VNA Care is Easy
It can be difficult to know where to turn when you or a loved one needs help with health care issues at home. The VNA is here to help.
Our Intake nurses will discuss your options, answer questions, arrange services, and clarify insurance coverage. We offer coordinated interdisciplinary services to address any home care needs, and work in partnership with referring physicians and other providers to deliver coordinated care from the simplest to the most complex cases. Thank you for considering the VNA for your home care needs.
Our Intake and Referral nurses are here to help you 24 hours a day, seven days a week. They will help you find the care you need, often initiating VNA services within 24 hours of the referral.
Our Insurance Specialists strive to ensure you are informed about the Home Care Benefit from your insurance company. Our team has established reliable, collaborative working relationships with insurance companies and will work diligently with your insurance company and VNA staff to identify what services are covered and what, if any, liability you may incur. And, our Insurance Specialists will obtain prior authorization if needed.

NEW! Face-to-Face Requirement
Does your patient or family member have Medicare? If so, you now need to have documentation from a physician of a face-to-face encounter in order for the referral to be processed. To do that, simply print out and complete the Face-to-Face form listed below. We've also included a guide to help you in the process of filling out the form, a fact sheet with more information about the form and some examples.
Your physician must sign and date the form before you send it back to us.

Different Ways to Make a Referral
We offer a variety of ways to make it easy for you to make a referral:
1. By Website
You can now fill out a secure online referral form by clicking here. You will need to fill out the Face-to-Face Encounter Certification Form that is posted below and fax it to 802 860-4464.
2. By Fax:
You may download a pdf version of our referral form, print it out and fax it back to the Intake office at 802 860-4464.
3. By Telephone:
Weekdays (8:00 am to 5:00 pm):
Call 802 860-4400 or toll free at 1-800-427-1908
Nights, Weekends and Holidays:
Call 802 658-1900 or toll free at 1-800-427-1908 and a VNA nurse will call you back.
We also offer an "after hours voicemail" for referrals:
If you prefer to leave a voicemail message for an Intake Nurse, please call 802 860-4484.
Please leave your referral request and an Intake Nurse will call you (or someone whom you designate) on the next business day to confirm acceptance of the referral. Please speak slowly and include the following information:
- Your Name,
- Telephone number where we can call you back,
- The name of the patient you are referring (please spell the name) and their date of birth,
- And, if you are calling from a physician's office or a hospital, your department or floor.
You may fax additional information to the Intake office at 802 860-4464.
4. By Email:
You can also contact us by email at email@example.com at any time and an Intake nurse will call you back by the next business day to discuss your needs.
