Long-Term Health Care
The goal of the VNA’s long-term care services is to help participants maintain independence in the setting they prefer, to enhance the quality of life for both the client and the caregiver and to acknowledge and value the unique contribution of each family member. These programs enable both client and caregiver to remain integrated and active, a vital part of their community.
When needed, the VNA’s long-term care services are available 24 hours a day, 7 days a week.
To be eligible for these services, you must be eligible for Medicaid. If you are not eligible for Medicaid, you may pay privately for services. Please visit our Private Care section.

Our long-term care services include:
- Choices for Care — working side by side with you to maximize your independence and quality of life.
- Traumatic Brain Injury Program — helping those who have suffered a traumatic brain injury retain their independence and help them maximize their potential level of functioning.
- Adult High-Tech Program — helping technology-dependent individuals live in their own home.
- South Burlington Community Housing — VNA staff provide support 24 hours a day at Anderson Parkway residence in South Burlington.
