We understand that it can be difficult to talk about end-of-life plans for yourself or your loved ones. We are here to help make it easier, with the knowledge, care, and compassion for helping you and your family understand your options and put a plan in place for your care.
Start the Conversation is a public education initiative that aims to:
- Increase the understanding of end-of-life options, including hospice and palliative care
- Help families talk with each other and their physician about end-of-life care
- Encourage the use of advance directives to document end-of-life care choices
Talking about end-of-life care early can help both you and your loved ones experience the care you want. An array of resources are available to encourage conversations about end-of-life care, such as care preferences questionnaires, conversation guides, community presentations, helpful facts, and a dedicated website to help patients and families access palliative and hospice care for themselves or a loved one.
