Our VNA Home Rehabilitation Therapy Services are there for you when you need us. Our services allow you to receive rehabilitation care in the comfort of your own home after hospitalization or as approved by your physician. We provide physical therapy, occupational therapy and speech therapy to people who have medical problems or other health related conditions that limit their ability to move and perform activities of daily living.

Our home therapy services help you stay in familiar and comfortable surroundings, thus promoting your independence, increasing your activity, and making it easier for your family to be involved in the healing process. Care from a VNA home rehabilitation therapist may reduce or eliminate the need for an extended hospital or inpatient rehab stay.
Home Rehabilitation Therapy Referral Options
Referrals are made by hospitals, extended care facilities, physician offices and other health professionals within the VNA. We also respond to self-referrals for home safety assessments.
