The VNA's home elder care specialists are skilled in planning care to meet your individual needs. We understand your desire to be as independent as possible and to remain in your own home. The VNA offers a wide range of elder care services to support independence at home, with extra help as needed. You may want your family members or other caregivers to be involved in planning, communication and giving care. VNA nurses, rehabilitation therapists, social workers and nursing assistants work with you and your family to provide the right care when it's needed. When the unexpected happens, a VNA nurse is always available to help decide how to manage the situation. We strive to protect your safety, health and comfort.
Home Safety and Fall Prevention
An expert physical therapist and/or occupational therapist will make a thorough assessment of your safety at home and develop an individual program to prevent falls and injury. Your care plan may include adding safety devices such as grab bars, learning safer ways to manage household activities, and doing home exercises to improve your strength and balance. A registered nurse may also be involved to help you safely manage your medications.
Dementia and Cognitive Impairment Care
Our VNA elder care team is experienced in all aspects of caring for clients with mild to severe memory loss or cognitive impairment. Based on an individual assessment, the care team may include a mental health registered nurse, physical therapist, occupational therapist, speech therapist, medical social worker, and nursing assistants. A plan of care is developed to maximize safety and quality of life for both you and your family. The care plan addresses your medical, social, and emotional needs, as well as providing support for your family in managing the stress of caregiving. Community resources are included, whenever appropriate.
Medication Management
Many seniors take multiple medications every day. Understanding the purpose of these medications, and keeping track of when to take all the pills, can become a challenge. A VNA elder care nurse can help sort out why, when and how to take all the medicines so that you can get the most benefit from them, and will know when to contact your health care provider about medication problems.
