Chronic Illness & Disease Management Services
The VNA offers skilled home care services to those learning to cope with the management of long-term disabilities or chronic illness. Our team of home care professionals works with you, your family and your doctor in developing and providing a comprehensive plan of care to allow you to stay in your home setting.
Whatever your need, the VNA has nurses available seven days a week, 24 hours a day, with day and evening scheduled visits and an on-call service at night.
You may also receive:
- Education relating to health, wellness and disease management
- Assistance with managing your medications
- Regular assessment of your current physical status and progress made
- Support in activities of daily living
- Care coordination services
- Referral to additional community services
Disease Management
The VNA offers a home disease management program that provides strategies to help you learn how to control the impact of conditions such as diabetes, asthma, cancer and HIV/AIDS. Care is provided not only to help you understand your illness but also to help you achieve the best health possible while living with your disease.
VNA Telehealth Program
The VNA's Home Care Telehealth Program monitors your vital signs and symptoms via a machine that transmits the information to the VNA. Telehealth monitoring has been shown to improve the effectiveness of home care and to decrease unplanned emergency visits and hospitalizations.
