Adult Home Care Services
VNA's Adult Home Care program provides skilled, Medicare-certified, state-of-the-art home care around Chittenden and Grand Isle Counties to promote health and minimize pain and suffering. We work collaboratively with you, your family and your doctor to develop a plan of care that will allow you to remain safe in your home and to be as independent and active as possible.
VNA care is coordinated by a case manager, who plans individualized home care, according to each client's health care needs and physician orders. Our team also includes a full range of home nursing and rehabilitation specialists. In addition, our nursing assistants provide help with bathing and other activities of daily living. Our social workers and mental health nurses provide counseling and referrals to community services.
Your VNA home care begins with an assessment visit by a registered nurse or physical therapist, who will talk with you to determine your needs and a plan of care. Other members of the team also will visit, as needed. Your case manager will continue to be available to answer your questions and adjust your care. Visits will continue until your goals are met.
Your VNA home care team members may include:
- Nurses, including experts in IV therapy, wounds and ostomies, mental health and rehabilitation
- Licensed nursing assistants
- Physical therapists
- Occupational therapists
- Speech therapists
- Medical social workers
- Registered dietician
It is the mission of the Adult Home Care Program as well as the Visiting Nurse Association to provide care to all members of the community who need our services.
